How do I change my mailing address?

To change your mailing address, please provide your current address by e-mailing zionsctops@zionsbank.com. Please include your account number, if known, social security number, and all names on the account with your current address.

It is important to notify us of address changes to avoid delays in receiving any mailings the Company may do.

How may I obtain information regarding my account?

Contact our transfer agent, Zions First National Bank (zionsctops@zionsbank.com) it is necessary for you to make your request in writing.

Please include your account number, if known, social security number, and all names on the account in your correspondence with your specific questions.

 

How do I replace a lost or stolen certificate(s)?

Please notify our transfer agent, Zions First National Bank (zionsctops@zionsbank.com).

Please include your account number, if known, social security number, all names listed on the account and a list of all original certificates you have in your possession in your correspondence.

 

How can I sell my shares?

We are listed on the Nasdaq Stock Exchange (SNFCA), therefore, any stock broker should be able to assist you in selling your shares.

If your certificates were issued 1987 or later, contact a broker of your choice and provide them with your original stock certificates. Please contact our transfer agent (zionsctops@zionsbank.com) to be certain that you have all stock certificates, issued to you, in your possession before taking them to a stock broker. Any lost or misplaced certificates not in your possession will remain in your account until replaced and sold.

If your certificates were issued previous to 1987, it is recommended that you bring your account current before contacting a broker. In order to bring your account current, please submit all original certificates to our transfer agent (zionsctops@zionsbank.com). When you receive the new certificate, then you may contact a broker of your choice and provide them with your original stock certificate.

 

How do I obtain the original price paid for my shares for tax reporting?

We recommend you contact a tax adviser. We do not maintain historical prices. Therefore, we are unable to advise you of the original purchase price of your stock.

How do I remove the name of a deceased Joint Tenant from the certificates?

Contact our transfer agent, Zions First National Bank (zionsctops@zionsbank.com) of their passing. They will provide you with the requirements to remove the decedents name.

 

How do I change my name on the certificates if my legal name has changed?

If your name has changed legally (such as through marriage), you must contact our transfer agent, Zions First National Bank (zionsctops@zionsbank.com).

 

How do I transfer the stock to another person?

What is a Medallion Signature Guarantee?

A Medallion Guarantee is a stamp and signature on a stock certificate or stock power obtained from an eligible institution, such as a commercial bank, broker, trust company, etc. It provides transfer agents assurance that the person signing a stock certificate or stock power is the authorized individual. A notary stamp is not acceptable when transferring the ownership of stock.
